This is your chance to help shape respite services for carers of people with disabilities in Kingston. The more carers and family members that fill in this questionnaire, the more informed our work will be.
If you would prefer to receive a phone call, please email us with your telephone number and the best time to call you between now and 11 November 2018. If you fill in this survey, or ask for a phone call after this time, we will still consider your response but it may not be part of the respite analysis. We hope to produce this analysis by the end of November 2018.
Respite, also known as carer relief or short breaks, is for eligible carers of people with disabilities. Carers' eligibility is assessed by the Kingston Carers Network or council officers.
Why are we asking you to fill in this questionnaire?
We would like to know what works well and what could be improved in order to make sure that the service we are providing meets everyone’s needs. If the respite currently provided isn’t working for everyone, we would like to know why.
The answers to this questionnaire will be kept anonymous. However, please leave your details at the end of the questionnaire if you'd like to be involved in further engagement events around respite and to hear feedback on the results.
